Book a Ride
1. Book your ride at least 2 working days before whenever possible.
2. Have the information ready:
a. Your date of birth, first and last name, and phone number.
b. Medicaid ID number.
c. Your doctor’s or facility’s name, address, and telephone number.
d. Type of service and time of appointment.
e. Your pick up and drop off address and details.
3. Yamhill CCO Members can pick one of the options to book your ride:
